Once the pop-up window is completed and saved, users will be taken to a new screen where they will see that a Waiver Form has been created.
Overview Tab
Users should first select the Overview tab and review the Instructions section for waivers.

Next, they should scroll down to the Justification for Late Submission section.
Justification for Late Submission or Outside of Current Fiscal Year Section
Users should use this section to provide information as to why their request is being submitted outside required regulatory timeframes; if applicable. If the request is being submitted for next fiscal year, please indicate so here.

When users complete this form, they should select the Save button.
