Navigating to the Intake Form
Users should navigate to the All Forms section on the Basic Field Grant Application Attachments tab and select the blue pencil icon to begin the Intake form.

As with the previous forms, the Intake Form contains parts that are necessary for application completion. Begin by answering each question, paying close attention to the red asterisk (*) for required questions.

Telephone Intake-Specifics and Walk-In/In Person Intake-Specifics Sections
In the Telephone Intake – Specifics section of the Intake form, users will need to document the days and times in which the organization conducts telephone intake for all phone numbers with which they handle LSC eligible calls.
Select the New button to begin.

Once the pop-up window appears, users should begin by entering the phone number, followed by the day of the week and the time of day that intake occurs on each number.

Users should select the Save button when complete.

Users should repeat the same process in the Walk-in/In-person Intake – Specifics section of the Intake form.
PLEASE NOTE: If there are multiple phone numbers, users will need to complete this form as many times as needed.
APPLICATION UPDATE:
This grant cycle a new feature has been introduced to assist with Telephone Intake and Walk-in/In-Person Intake data entry. Users may now utilize the Copy Walk In Details button or Copy Telephone Details button to copy hours of operation from one office to another or one phone line to another.

A new pop-up window will appear with a listing of any previously recorded operational hours for your organization’s telephone line or walk-in/in-person services.
Users should place a checkmark next to each of the entries that they wish to copy forward to the current Basic Field Grant application to reflect the operational hours of their telephone line or walk-in/in-person times. When all data entries are selected, users should select the Add button.

When data is entered for all sections of the form, users should select the Save button.

Next, users will select the Validate Form button to find missing or incomplete questions. If validation error messages appear, users should answer or rectify the errors and then select the Save button again.

After validating the Intake form, users may select the Back button to return to the main screen, or they may select the next form from the drop-down menu in the top right-hand corner of the screen and be taken to another form.
